Read in full. This agreement is between the parent or legal guardian of the participant and Elite College Identification Camps, Inc., doing business as Refine Elite College ID Camps
1. Assumption of risk. I understand that soccer is a physically demanding contact sport and that participation in a Elite College Identification Camps, Inc., doing business as Refine Elite College ID Camps ("RECIC") camp involves inherent risks of injury, illness, permanent disability, and in rare cases death. These risks include, without limitation: collisions with other participants, the ball, goals, or fixed objects; falls; sprains, strains, fractures, dental and facial injuries; concussion and other head, neck, or spinal injury; overuse injury; heat illness, dehydration, hypothermia, sunburn, and other weather-related conditions; lightning; poor or changing field conditions; equipment failure; exposure to communicable illness; allergic reaction; aggravation of a pre-existing condition; and injury sustained travelling to or from the camp. I understand these risks cannot be eliminated regardless of the care taken by RECIC, and I knowingly and voluntarily accept them on behalf of the participant.
2. Physical fitness and medical disclosure. I certify that the participant is in good health, is physically fit to take part in strenuous athletic activity, and has no medical condition that would make participation unsafe. I have disclosed all allergies, medical conditions, medications, and prior injuries relevant to participation on this registration form. I will notify RECIC promptly if the participant's condition changes before camp.
3. Medical treatment authorization. I authorize RECIC staff, coaches, volunteers, athletic trainers, and emergency medical personnel to provide or arrange first aid, medical care, transportation, and emergency treatment — including hospitalization, anaesthesia, and surgery — for the participant if I cannot be reached. I accept financial responsibility for all costs of such care, including ambulance and hospital charges. I confirm that my own health insurance is the primary coverage for the participant and that RECIC provides no medical or accident insurance.
4. Concussion and injury protocol. I understand that any participant suspected of sustaining a concussion or other significant injury will be removed from activity, and that RECIC may require written medical clearance before that participant returns to play. No refund is due for time missed as a result.
5. Release and waiver of claims. In consideration of the participant being permitted to take part, I, on behalf of myself, the participant, and our heirs, executors, administrators, and assigns, hereby RELEASE, WAIVE, and DISCHARGE Elite College Identification Camps, Inc., doing business as Refine Elite College ID Camps, its officers, directors, board members, employees, coaches, instructors, volunteers, contractors, partner clubs, attending college coaching staffs, host facilities, property owners, sponsors, and agents (collectively the "Released Parties") from any and all claims, demands, damages, actions, and causes of action arising out of or related to any loss, damage, injury, illness, or death sustained in connection with the camp — including any caused by the ordinary negligence of the Released Parties — to the fullest extent permitted by law. This release does not apply to gross negligence, recklessness, or wilful misconduct.
6. Indemnification. I agree to indemnify and hold the Released Parties harmless from any loss, liability, damage, or cost, including reasonable attorneys' fees, they may incur as a result of the participant's involvement in the camp or arising from any claim brought by or on behalf of the participant.
7. Personal property. RECIC is not responsible for loss of or damage to personal property brought to camp.
8. Conduct. I understand that participants are expected to conduct themselves respectfully toward staff, coaches, officials, facilities, and other participants, and that RECIC may remove a participant for conduct that endangers others or materially disrupts the camp, without refund.
9. Photography and media. I grant RECIC the irrevocable right to photograph, film, and record the participant at camp, and to use those images, recordings, and the participant's name and likeness for promotional, educational, and recruiting purposes, without compensation.
10. Severability and governing law. If any provision of this agreement is held invalid or unenforceable, the remaining provisions remain in full force. This agreement is governed by the laws of the State of Missouri, without regard to conflict-of-law principles.
11. Acknowledgement. I have read this agreement in full, understand that it releases legal rights and is a contract between me and RECIC, and sign it freely and voluntarily. I am the parent or legal guardian of the participant, or the participant is 18 years of age or older and signing on their own behalf.
